Filing an Appeal - Public Benefits
To appeal a decision by the County Department or State Agency regarding Medicaid or other public benefits, you may request a hearing with the OAC by either:
- Filing a Request for State Level Hearing form,
- Writing a letter to the OAC requesting a hearing, OR
- By calling the OAC at 303-866-5626.
Your request should include the following information:
- Your name, address, phone number, and email address.
- The name of the agency or department that took the action you disagree with.
- The program(s) involved.
- The name, address and phone number of anyone who will be representing you at the hearing.
- Please include copies of any denial/suspension/termination notices you have received.
You can submit your hearing request to the OAC by email, U.S. Mail, E-filing, fax, or hand delivery.

SNAP Benefit Appeals
The OAC no longer hears appeals for Supplemental Nutrition Assistance Program(SNAP) benefits, unless the case involves Intentional program Violations (fraud). Appeals for SNAP benefits are handled by the Colorado Department of Human Services, SNAP Hearing Unit, 3550 W. Oxford Avenue, Denver, CO 80236.
