OAC Forms
The Office of Administrative Courts has a wide variety of forms to assist parties in providing the Court information we need to process your appeal or hearing request. Depending on the type of case, some forms are required.
All non workers' compensation forms (General Services) can be found in OAC e-filing system. General Services forms can be submitted electronically through OAC e-filing.

Name Form Description Revision date Amended Application for Hearing PDF Word Use this form to make changes to an Application for Hearing that has already been filed. 9/2023 Application for Hearing PDF Word This form is used to request a hearing in a workers' compensation claim 9/2023 Application for Disfigurement Hearing PDF Word Use this form to request that an Administrative Law Judge evaluate a work-related disfigurement. 9/2024 Application for Expedited Hearing PDF Word This form is used to request an expedited hearing. The situations that qualify for an expedited hearing are listed on the form. 5/2025 Audio Request Form PDF Word Use this form to request a copy of the audio recording of your hearing, or to have the recording sent to a court reporter who will prepare a written transcript of the hearing. 5/2025 Cancel Hearing Form PDF Word Use this form to cancel a scheduled hearing. 3/2015 Case Information Sheet PDF Word This form is used to advise the ALJ about the status of a scheduled hearing, including the issues to be addressed and any witnesses a party plans to call. 9/2023 Hearing Confirmation Form PDF Word Use this form to confirm the date and time the parties have selected for the hearing in a workers' compensation case. 6/2024 Entry of Appearance Form PDF Word This form is used by attorney's to identify the party or parties they are representing in the case. 5/2025 Interpreter Request PDF Word This form is used to request an interpreter for a hearing. You can request if you have limited English skills or need accommodations for a disability. Please submit this form as soon as possible after your hearing date is confirmed to allow enough time to make arrangements. 8/2019 Order Status Request PDF Word Use this form to request a status of any order to be issued in association with the claim. 2/2022 Petition to Review PDF Word Use this form to appeal a decision of an ALJ after a hearing, when a transcript is not being requested. 6/2024 Petition to Review and Transcript PDF Word Use this form to appeal a decision of an ALJ after a hearing, and to request a transcript of the hearing. 6/2024 Request Specific Findings PDF Word If the ALJ issued a Summary Order after a hearing, use this form to request a full Order. 3/2015 Response to Application for Hearing PDF Word This form is sued by the opposing party to add issues and witnesses after an Application for Hearing has been filed. 1/2024 Subpoena PDF Word Use this form to request that an ALJ approve a subpoena for a witness to testify at hearing. The requesting party is responsible for serving the subpoena on the witness. 5/2025

Name Form Description Revision Date Audio Request Form Word Use this form to request a recording of your hearing for yourself or for a court reporter who will prepare a written transcript. 4/2024 Health First Colorado Non-Attorney Authorization Word A party in a Medicaid case must complete this form if someone other than an attorney will be helping them with the appeal. 3/2017 Request for State Level Hearing PDF If you disagree with a decision about your public benefits, use this form to request a hearing with the Office of Administrative Courts. 7/2024 Representative Authorization Word You must complete this form if someone other than an attorney will be assisting you during the Appeal Process. This can be used for all public benefits cases EXCEPT Medicaid appeals. 3/2022 Subpoena Word Use this form to request that a judge approve a subpoena for a witness to testify at hearing.
